On-line Arrangement Process
First Contact
When a death occurs, contact Sage Funeral Home by phone to notify us and authorize the transfer of your loved one into our care.

Confirmation & Instructions
Once your loved one has arrived safely at Sage, a funeral director will contact you to review their wishes and go over the on-Line arrangement process.

Vital Statistics
Your funeral director will send a link to complete a Vital Statistics form online. From the comfort of your home and at your pace, the Authorized Decision Maker will complete the Vital Statistic Form & Authorized Decision Maker Declaration.
Identification of the Deceased
Your funeral director will arrange & coordinate with you for the mandatory visual identification of your loved one.

Authorization of Services
Your funeral director will prepare all required documents and send electronically for review and signatures by the Authorized Decision Maker.
Return Home
Following cremation, your funeral director will maintain communication with you. Once we have your loved one in our care from the crematorium, we will arrange to have your loved ones ashes returned to you.
