What We'll Ask You
About you
Your full legal name, date and place of birth, home address, marital status and your spouse's full name, your main occupation, and your family doctor.
Your parents
Your mother's full name, including her maiden name, and where she was born. Your father's full name and where he was born.
Your wishes
Burial or cremation, the kind of service or gathering you'd like, music, readings, clergy or a celebrant, and details for your obituary, along with a favourite photo.
Keep with your plan
Your Social Insurance Number and Saskatchewan Health Card number. Your family will need them at the time of death, so it's best to keep them with your plan documents.

Pre-Planning Questions
Is there a cost to pre-plan?
No. There's no cost to sit down with us, talk through your wishes and have them placed on file with Sage.
Is my price guaranteed?
Yes. When you fund your plan, we write a guarantee and hold your price, so your family won't pay more later.
Can I pay monthly?
Yes. Pre-arrangements are funded through Purple Shield funeral insurance, with lump-sum and monthly payment options.
What if I move away?
Your plan moves with you. You'll re-plan with a funeral home where you live, and your funds can be transferred.
Can I change my plan later?
Yes. You can update your plan at any time, and we'll help whenever you need to revisit it.
I already have a plan with another funeral home. Can I move it to Sage?
Yes. Saskatchewan law lets you cancel a pre-need contract at any time under Section 81 of The Funeral and Cremation Services Act, and we'll help you transfer your plan.
